To me it's most likely los padres y dos hijos or los padresy dos hijas depending on ( see my example)

From what i can see it's a mom and dad with a child. Based on the options their most be a second child in the photo we can't see. So if both of the children are girls the answer is los padres y dos hijas, but if one child is a boy then it's los padres y dos hijos.
