Awaiver of the requirement for documentation of informed consent may be granted when:

A waiver of the requirement for documentation of informed consent may be granted during some of the following : conducting research examining state and/or public service programs; performing emergency research; research involving children; breach of confidentiality risks; minimal risk of harm to the subject; and/or research is conducted by state or local government officials.  

A waiver of the requirement for documentation of informed consent may be granted when the only record found linking the subject and the research is the consent document that is requested.

Explanation: Informed consent is when permission is granted even when the consequences are known and may vary in their cases. If a doctor was giving a patient medication that has both good and bad side effects, but the patient agrees knowing both, this is an informed consent. They understand what there can be benefits and risks but are moving forward regardless.
