• Vision coverage that covers one eye exam and up $150 for glasses or contact lenses

The basic health and major medical insurance includes a $250 deductible, 80/20
coinsurance, up to $500 preventative care paid at regular rate (no preventative care coverage over $500), $7,000 out of pocket maximum.
