What lessons can be learned about city cleanliness? (when examining the middle ages.)

this is a problem faced by all over the world...mostly it is because there isnt proper waste management or it could be because there is less trash bins in the street..so there are lots and lots of cons for not miantaining cleanliness .so we shouldnt do this..
