a. Discuss the potential disease processes and signs and symptoms related to the disease process that Jose may be exhibiting?
b. How would you provide multidimensional care to include cultural, psychosocial, spiritual, physical, and emotional needs for Jose?

The given data suggests that the client Jose is having edema of the scrotum, urinary retention and haematuria. The rectal examination shows prostate enlargement.

This suggests that the client having Benign prostatic hyperplasia.

---Benign prostatic hyperplasia. It is also called benign prostatic hypertrophy and enlargement of the prostate gland. The prostate enlarges and moves upward towards the bladder that results in obstructs the outflow of urine. The urine collected in the bladder and results in infection.

The signs and symptoms are:-

- increased frequency of urination.

- urinary urgency is common.

- obstruction causes inability to start and continue a urinary stream

- the bladder detention is seen.

- Have presence of blood in urine.

- an incomplete bladder emptying.

- Bladder pain occurs due to retention.

- Urinary tract infection.

- Renal failure.

The management includes catheterization, increasing fluid intake and surgery.

b. The multi dimensional care is needed for the client.

- Monitor vital signs to find problems.

-The client have anxiety. Explain the disease condition, treatment, complications.

clarify the doubts of client to reduce stress and anxiety.

-Encourage to take bed rest.

--Encourage to take plenty of oral fluids to prevent dehydration.

- Encourage to take small and frequent foods.

--Administer analgesics to reduce pain.

- Explain the need for sitz bath and how to do it.

- Encourage to avoid strenuous activities.

- Encourage to avoid prolonged sitting.

- Avoid excessive straining for defecation. provide enema if needed.

- Explain that haemorrhage is seen after surgery and to inform if excessive bleeding occurs.

- Advice to report for any signs of infection like fever, chills etc.

--Perform aseptic methods for care giving to prevent infection.

- Explain that after catheterization removal time is needed to reach normal bladder control.

Dribbling of urine is seen. Encourage to use pads.

- sexual dysfunction is seen. Explain that it is common and will become normal gradually.

- Reassure the client.

- Encourage family members to support the client and to provide care.

Benign prostatic hyperplasia can be corrected gradually. so must provide support and care to the client.
