1. What types of information would be asked/required for children's health records?

Name, address and phone number of current primary care physician/dentist, as well their immunization records (or a waiver signed by the parents about the immunizations),  medical records, allergies, diet restrictions, current conditions, pretty much what you wish to know beforehand of like a general story of a child.
