Contributing factors that lead to human rights violations

Some of the contributing factors that cause human rights violations are weak civilian evaluation or when civilian agencies are not used by the public and are attacked by police and armed forces.  Bad leadership which fails to put a stop to abuses.  Ineffective solutions that do not stop abuses but are blocked by corrupt police and lenient punishment where cops get off easily.
