Explanation:

1: Integral membrane proteins are usually spans the whole membrane and these type of integral proteins are "transmembrane" proteins. To form a channel they need to span lipid bilayer (two layers) and needs to be attached to it all the time (see first figure). Ion channels are no exception.

2: If it a sodium channel is always open it means it has no gate (non-gated) and allows the ions to travel freely (see second figure).

3: To create the action potential, the electrical current that allows neurons to pass on the information ion channels pumps the sodium ions inside and the potassium ions outside (see third figure)
